Forms for health insurance

The ‘new health insurance’ for pensioners, including spouse/family pensioners, is to be implemented with effect from July 1, 2014.For this, details of pensioners and their spouses and family pensioners have to be collected in a prescribed format.
The forms will be issued to the pensioner/family pensioner by the pension pay office/treasury/public sector bank at the time of mustering.The filled-in forms with photographs and proof of date of birth of spouse may be given to the pension pay office/treasury/public sector bank either in person or through messenger before June 30, 2014.
The pensioners/family pensioners shall take a copy of the filled-in forms and get an attestation from the pension disbursing officer concerned.The forms can also be downloaded from the websites www.tn.gov.in or www.karuvoolam.in.
