Visit the nearest Government Health Hospital. Get the application. Fill it up and get it attested by the VAO (Village Administration Officer).
Then, take the filled and attested application to the Office of the District Collector and show the ration card and Aadhar card for confirmation and obtain the 22-digit Insurance Coverage number.
You may use this 22-digit insurance coverage number in the hospitals for treatment.
Visit the nearest E-Service Centre after 3 months and receive Insurance Coverage Card with a photograph!
