If the original insurance policy is lost, one needs to redeem the same with a fresh one!
Who to approach for this? The branch which distributed the policy!
What are the documents required?
- Address Proof
- Copies of Photos signed by the notary public
- A copy of any of the receipts for having paid the premium
What are the charges?
- Rs. 75 is to be paid for the compilation of documents.
- Apart from this, charges at the rate of 20 Paise for every Rs. 1000 of the coverage amount has to be paid.
Time for action: The fresh policy may be issued within 15 days of application.
Procedure:
- After submitting the application needing a new policy as the original policy has been lost/misplaced, the branch will issue 2 documents.
- Out of this, one document has to be printed in an Rs. 80 stamp paper.
- On the 2nd document, there would be questions asking for the details of the lost policy. This has to be filled up and submitted after attaching all required documents signed by a notary public.
